To apply while the waitlist is open, pick up an application in person at the BMHA Office at 4110 Hamilton-Middletown Rd. Hamilton, Ohio. Monday-Thursday, 8:00 am until 4:30 pm.

Once the paper application has been completed, it must be: returned to the BMHA office, located at the address mentioned above.
